Prior to making your appointment you will need to complete the intake paperwork. You will be emailed a link or you can completed the forms from the website. There is additional paperwork to be completed upon arrival to your appointment and we strive to make the most use of our time together.


We receive a high volume of phone calls each day. Please leave a message and your call will be returned as soon as possible. We strive to return phone calls within 24 hours but typically return them in a few hours. Messages left on Fridays may not be returned until Monday.


Both biological parents will need to sign permission for the psychological assessment. Only one parent needs to attend the appointment but both will need to sign the permission form. If one parent has sole custody legal documentation will need to be provided prior to your appointment. If you have any questions about permission for minors please call 480-659-5563.


We strive to see individuals and their families as quickly as possible. Each appointment is a minimum of 2 hours for your individual assessment. If you no longer need your appointment please let us know as soon as possible so we may meet the needs of another individual. If you or your child is sick the day of your appointment please let us know and we will rescheduled. No show appointments will not be rescheduled.


We only provide psychological evaluations to help individuals and families receive accurate identification of psychological issues in order to develop appropriate and effective treatment plans. Many services that may be of assistance to individuals are not accessible without a psychological evaluation. We will put together recommendations and provide referrals based on the assessment results. We do not provide any counseling services.
